Are they, though? Isn’t it a just through marriage thing since Alex’s mother is Anjelica and not Adrienne? I mean just that there’s no physical blood relation. They likely grew up together as cousins in the family.

Xander and Sarah. But they are not in the younger age group.

The bed hopping with these characters means I have no faith in any longevity in any of the pairings.

This is correct. Anjelica faked her death and Adrienne was Alex's mom from the time he was a year old. He grew up with Sonny and the twins as his brothers. Both he and Stephanie joked about their mutual Grandma Jo: Adrienne's mother!
 
Bad enough that Max hooked up with Stephanie back in the day, because they literally spent a lot of time together as kids. (Max and Stephanie are legal uncle and niece) Max and Chelsea, while still gross, is a little better because Chelsea didn't find out she was a Brady until she was a teenager, so she and Max didn't grow up together.

With re-Ron's thinking, anything could go. But I'd strongly object if they were romantically interested in Stephanie/Joey, Abby/JJ, Shawn-D/Ciara/Claire, Brady, or Philip (I just included all genders for ease).

These characters also have no baggage, no backstory other than the drug thing they developed in Arizona. They could be wacky and nobody would say "this isn't Joseph!!" They could be evil, angry that their father replaced their mother with a lookalike. Heck if they had a phony will or even a real codicil to the will that gave them a piece of Titan, it would be more interesting than Philip failing up again (and battling Xander again).

Okay, moon-eyed speculation over.
